I have been a patient at the Glendale Wisconsin center for awhile now. This medication has LITERALLY saved my life! I cannot even begin to express how FANTASTIC the staff and counselors are. From the time I walk in the door, the check-in, lab testing, seeing a counselor and then checking out...EVERYONE is so nice and helpful. Also, any time that I have had to call because of a question or something, the customer service is GREAT. They are patient and helpful. There is not 1 thing bad or negative that I can say about any of my experiences there.

I've been going to cleanslate in glendale since 2021. While I've had a couple different NP's, I've always been treated with fairness and compassion, they have made themselves available when my pharmacy makes mistakes, which is often. I trust their staff. My experience with Cleanslate has been life changing.Edit 12/18Aaron went above and beyond today. Understaffed, lots of clients, he was kind and helpful, understanding, he made sure i was seen by MY provider when there were issues with the equipment. Aaron is a superstar.

The staff was very friendly. The doctor took the time to explain the treatment plan with me. I was able to get the help I needed in timely manner without a long wait in the waiting room. Overall, I really enjoyed my first visit.

This place has been a consistent force for good in my life. I’ve been to other clinics that offer the same services and you usually never know what you’re going to get, and who you’re going to deal with. CleanSlate holds you to your recovery without making you feel like a criminal, and they take the time to get to know you and therefore how they can help you best.

Worst place!!! A few nice people but other that they should be fired. I went in on thursday afternoon. I just started monthly visits (which clean slate doesn't make the kind of money when someone goes twice a week). I dealt with a new person on thursday and all went fine.Thursday night script was never sent, friday the place is open half day was told my script would be sent. NEVER was. Saturday came and I called at 9am right when the place opened and was told my script will be at the pharmacy in 30-40 mins. I called 90 mins later and STILL not there. I called back talk to a guy named Diego who said my script will be there by today. He actually said I PROMISE!I called at 11:41am to make sure and the woman said it was after hours. I said no um it isn't noon yet and she said yes it is well after noon. Come to find out I talking to a woman that lives in New Hampshire?!?! She proceeds to tell me I should go to an emergency room?!?!?! An emergency room? What do I tell the ER doc I need my script? That is going to go over well. Oh, and most are not sub docs. This whole thing is ridiculous.How hard is it in THREE days to send in a script on a laptop. When Sally Vahovick left, this place when it comes to filling scripts has gone to hell. Never had this happen when sally was there.UPDATE: 6 months later,This has happened AGAIN! On monthly visits. Script sent in the on Thursday. My pharmacy was until monday. So they told me to call around and find another place. I called 6 places and found one place and they told me to have a doctor send in a new script. I called on Friday at 8am. All day nothing! Called at 4pm. Place is closed and the answering service person said go to the ER! Um, they don't give suboxone at Emergency rooms. Then I well just fill it. I talked to 6 people from clean slate answering service and was told multiple things that all conflicted with what the others said. I come in do exactly what they ask and it is like pulling teeth to get anyone to not just do a decent job but anything at all.Why does it take days to send cancel and send in a new script!?!I can get better service at amazon when I order pencils. This is daily medication that people NEED!

Yes the staff is great. My issue is when i have an appt on friday morning the day the office closes early at 2pm but they call my script in rihht before they clise and i comento find out that my pharmacy is out of stock!! I call over and ovee to get in sent somewhere else but they didnt. So now im going through withdrawal all damn weekend!
