The people working here were extremely helpful in furnishing documents and describing what documents are and are not available. The building is hidden from the street; drive into the entrance between Lowe's and Pizza Hut in the back on the left, and look for the blue awning.

Niki at the desk seemed irritated by questions on the required form I needed to complete to pick up my medical records. She let out a huge sigh and was generally rude. I was honestly uncomfortable being there.
