Great place to go if your in pain. Dr. Jacob's REALLY knows what he is doing as well as the rest of the crew. I had pain relief after the first treatment and it just got better from there. I've avoided neck surgery which I was told by 2 different orthopedic surgeons I needed. The front desk girls are super kind and very professional. Both of my adult children now go here as well and have been very pleased with their treatments. I highly recommend FreeForm Chiropractic.

Dr. J and his team are truly the best. They go above and beyond for their patients and exude professionalism with strong expertise in a relaxed and welcoming environment. Highly recommend! Also, love all the sweet receptionist who are always so warm and inviting. ☺️

FreeForm is amazing. I was having constant neck and back pain and I am feeling a lot better after the sessions. The customer service is excellent! I do not feel like I am just another number.

I am SO IMPRESSED !!!Wow! What is happening here? 2 doctors on my case, they spend so much time getting to understand how my injuries have effected my life, the level of Empathy and Compassion is UNPARALLELED!!!Beautiful new facility is equipped with high tech equipment incredibly friendly and helpful staff, I can not say enough good things about my experiences here to fully make my point but, what I can say is that at FreeForm Chiropractic, it’s a new day in health/body/mind/spirituality, and I’m HERE FOR IT!

So thankful I found Freeform. All the doctors, interns, and staff are exceptional. They always go out of their way to make patient’s comfortable and make sure the care plan is explained and understood. As a 10+ year chiropractic patient, I wish I would have found Freeform sooner.

Man the caliber this place bring is unbeatable I got in a bad car wreck and dr. Jacob's walk me threw the whole process.Made it seamless and very simple but the experience this place bring is above and beyond they run high efficiency and super smooth.You walk in the front lady's and charity us so warming and welcoming.The guy in the back are so nice and informative and helpful.I wasn't fully convinced on chiropractor but they changed my mind on it, it does workAnd there top notch on working with availability they open 20 mins early to accommodate my schedule and work into their lunch.I appreciate all you in helping me in a hard time in my life thank all you for y'all help

I've known Dr. Jacobs for a long time now and he has the best practice and takes the best care of his patients. I am so grateful for the care I receive there on a weekly bases. Dr's Hildebrand, Rogers, and Fant each have unique healing hands that are able to turn on the power of your body for optimal healing. I can't end this without saying how very nice Charity and Diandra are. As soon as you walk in the door they are greeting you with big smiles letting you know you are in the right place. Amazing practice all around from start to finish.

I absolutely enjoy making FreeForm my first stop in the mornings before work. Everyone says hello and smiles which is a breath of fresh air these days. Between Dr. Rodgers, Dr. Fant and Dr. Hildebrand I have never felt like a doctor has cared for me or any of the other patients the way they do. I can't say enough good things about this office.

Jesse and the team are always friendly and welcoming! I started going to Freeform when I was 7 months pregnant. I didn’t realize what I been missing out on. After getting adjusted I felt so much relief in my lower back which is really important for ladies during pregnancy

Through my years of going to chiropractors, it has been many, I have only experienced the quality care from FreeForm that I am receiving at this time. I have explored Chiropractic care several times over the years. I always felt as though those offices were just trying to milk as much money as possible from me. There was no plan shared with me for my treatment. FreeForm has been different. The treatment certainly is not free, and my insurance only pays a portion. However, they were very up front with me about the treatment plan and cost associate with it, as well as what my goals needed to be.I can definitely say that after receiving treatment for the last 3 months that I feel better than I have in years. The issues I sought treatment for have been completely resolved. The entire staff has been very welcoming, professional, and cordial. I highly recommend FreeForm.
