My family member has been a resident at Gracedale for the past three months and has gotten excellent care. The staff is friendly and hardworking. I'm extremely grateful to them.

Severely understaffed. Will let one cna take care of 40 people. HR is a nightmare. Stay away from this place!

The amenities are amazing, excellent food and two activities per day, as well as a beautiful outdoor area, and even small garden. However, the lack of concern, empathy and attention to detail are terrifying for family members, bringing up worries of negligence and lack of care. Visiting my grandmother on Friday she was alert, capable of speaking in complete sentences, fully aware of where she was and her surroundings, and capable of feeding herself. On Sunday morning we had to ask about her complete change in well-being; incapable of stringing together a sentence, confused about where she was, and unable to use a fork or even lift her cup to feed herself. We heard five different stories about a near fall incident, and were told the nurses didn't notice any major changes... How is going from being able to speak and feed oneself to not being able to do either not a major change noticed by nurses?

Horrible staff. Horrible the way they treat each other and horrible how they treat their employees.

While some staffing is polite and inviting, there are others who do not take courtesy in the residents or the resident's family. My family member has been here since November of 2022. I dislike seeing my members' requests being disregarded, ignored, and sometimes outrightneglected.

This place is HORRIBLE, smelly and unsupervised, at least when 4 of my Close family members were placed there. Staff changed diapers with ICECOLD water. There was always an issue. My father had bruises, his belongings went missing and NOONE knew anything when questioned!

If I could give zero stars i would. Staff is not friendly place is not staffed nor has the necessities to take care of its population. Agency staffing is the Only thing keeping them in business.

My father was there for only a short time, but he was well taken care of, regular showers, always clean. The staff and nurses were awesome. They called me with updates on a regular basis and even helped him when he needed to use the phone. All in all, I was very satisfied with his care.

My mother has been a resident for 7 years. In the beginning her dementia wasn’t as bad as it was near the end. She was in the hospital many times broken wrist, fractured neck, laceration, black eyes due to falls and dehydration. There were only two nurses that I felt I had a rapport with. Needless to say. When my mom passed away I was not able to go into her room to to collect her things the door was padlocked. Her clothes were on a cart shoved in multiple bags. Didn’t even receive a sympathy note from them just final invoice. There is so much more I can say, maybe I’ll just save it for the law.

So. the D.O.N and the supervisors don't want to deal with anything or listen to the workers express concerns, but the STATE will! Happy New year!

Residence are well taken care of . Staff on the other hand not so much. Upper management shows favoritism toward some staff which is not okay everyone should be treated equally . Agency staff are always being talked about and truly under appreciated. Grown adults acting like children and it’s sad.
