Reta Holder is very rude. I called to let her know that she had a package delivered to my house by mistake and didn't even bother to call me back. I ended up calling FedEx Ground who made the mistake and they came by to pick it up.

"The food is good and the people are nice" is what I was told and it was certainly true. This was my dads 2nd round at skilled nursing. Therapy came at regular intervals, which was not our experience at the last place. Highly recommend it. We liked it so much he moved right into the Assisted living on the same campus upon discharge. If you are looking at reviews of facilities, I recommend ignoring negative ones from pandemic era as all these places struggled likewise.

I have worked here off and on in the assisted living side for 14/15 years. The amount of turnover is unsafe for resident care. Management does not put the residents first and neglects their concerns when they are brought up. The building currently has no nurse in the assisted living side which also affects the residents’s cares.Do your research before moving your loved one here.

I worked there as a CNA in the skilled unit for 13 years and I had to quit because the care there was awful. Anything staff needed or "complained" about was always our problem and never addressed, even if it was concerning patient safety. They constantly were running us short staffed and management would not help us out even when asked. At the time this was the only care facility I had worked at and didn't know any better, everyone told me "this is how it is at all facilities"Well that is not true. This building hasn't been decent since both Cheryl and Sandi S left.

They are horrible they give my grandpa chunks of food but he had to be on a liquid diet and when we said it they got mad and give him a protein shake instead of giving him real food

My Grandfather is a patient at avamere in Keizer. Do not take your loved one here! My grandfather has been neglected, no water or anything to drink for the entire day. They are not giving my 92 year old grandfather his pain medication. And moving him causing severe and extreme pain. Another patient said he was screaming for over three hours. They need to fire the care givers that are doing this!!

This has nothing to do with the caregivers at Avamere. They were always very polite and treated my mother very well. They genuinely cared for her. This is about their business office and billing practices. My mother passed away at the beginning of the month. I paid for her portion of her care each month, in advance. They would automatically charge my credit card and other than them raising the price on me without notification, it was fairly pain free. After my mother passed I asked about the billing policy and what I needed to do to make sure I wasn't charged the following month. They said that wouldn't be a problem and that I would also be getting a prorated refund from them. That never happened. After waiting and calling and never getting a call back, I guess I finally bugged them enough to respond. They left me a VM. It was short: "You are not entitled to a refund. Even though your mother did not stay with us for the entire month (only days actually), we do not offer refunds. We should not have told you you would be getting one. If this answer is not good enough for you (she literally said this in the VM), take it up with Medicaid." So, that's it. How much money do you think they get to keep by charging patient's families for an entire month after they've passed? I'm guessing it's considerable when you think of how many patients they have there. The unfortunate thing is that this is probably pretty common within the industry. They're out to make a buck any way they can even if it's absolutely unethical. They simply do not care. Please beware of them.

We have been both unhappy and happy with information provided regarding ongoing care, on plan of treatment, how long Ted is expected to have to be in the facility, the food, the treatment by some staff and ability to keep in contact with each other.The phone in his room does not connect to incoming calls so we have to leave a message with some one to tell him we called.He has been left for several hours in soiled underwear which is not a very good idea.No one has told the family what is happening with his progress, details on medical appointments or his progress or proposed further treatment, how long his stay might be, or just about anything.We have both had our COVID vaccines and it is now past two weeks since the second so why am I still not able to see him. Is there no way he can meet me outside or something?We are very disappointed and challenged wit the situation as it stands.Jill Rickman, 3/26/2021

This place is terrible place! They don't take care of their patience. My mother's hair was matted and they couldn't brush it out so they cut it she was throwing up and they took forever to do something about it , they didn't care about her or care about her diet she is diabetic and overweight. She even told me she hated it there.

My father is in rehab there now and they are very slow to respond to his needs and one of the attendants told me I should not be vising now because they said it was not a good time at 930 am . Took them well over 30 minutes to respond to the call button for help to the bathroom and to bed. Update we had a meeting today with the director and he was quick to address the issues .

This place is not a safe place for nothing. It was a very bad experience on my grandmother and would not wish this on anyone. No matter the feedback of the staff should you believe it. This doesn't even deserve any stars. The nurses wouldn't come for about 30 minutes after she pushed the EMERGENCY BUTTON and this is NOT OK. These people deserve to be fired and treated the same way. You try having a wet diaper for hours. NEVER AGAIN AND DONT TRUST IT FOR YOUR FAMILY!!!This was torture and supposedly for any person with dietary restrictions it is "DISCRIMINATION" to aide to those restrictions.Not good for aftercare or for a home facility. This is not a safe place whatsoever. My grandmothers patient next door to her even pressed her button and it took more than 30 minutes for the both of them.This place is DISGUSTING, DISRESPECTFUL, and INHUMANE.

From the beginng I found Avamere to be a nightmare. I was kept awake by nurses outside my door, gossiping loud enough to wake the dead. Needless to say, they got you up at 6:00 a.m. for "vitals" and then disappeared while you tried to go back to sleep. I had a hip replacement that bled during the night and filled the sheet with blood. They did nothing but change the sheets. You couldn't go to the bathroom without one of the staff hovering around to make sure you didn't fall. You were supposed to call them so they could be there. You could not get out of bed to go by yourself. Once I waited up to an hour to get relief. Women were showered by men without being given a choice. The worst of it was an aid who got her face in mine at 2:00 a.m. and yelled in my face, asking if I had to go to the bathroom. I had been fast asleep. After that she continued to bully me all the time I was there. I consider her actions to be elder abuse. So forget your placating little response to this review. I'm considering a lawsuit.

First time I put my trust into Avamere they took great care of my Grandmother. Second time was shocking to hear and see. They did not take care of her hygiene well. She was left in dirty clothes for multiple days and I'm not talking small stains. I had called and was very nice and asked why this was happening and i did not get a call back. Also i was told on multiple occasions she would be helped into the bathroom and then be left in there for 20 minutes or longer, so long that my dad and my uncle had to help her out because no one came back. What the hell Avamere? You charge $12,000 for this type of care? Is that how you want to be treated when you're elderly? Do not go into this type of career field if you cannot act decent and treat people correctly. I cant believe this.

We had an awful experience while my mother was at AvaNightmere. The first night she was there it took over 30 minutes to get someone to come into her room to help her move to the restroom. She called me and I had to call the front desk to get them to have someone go in and help her. The next day I was there and it took close to 20 minutes for me to find a staff member to assist her. Another afternoon she was left on the bedside commode for 45 minutes. We were told “we would like to respond to the call button within 5 minutes but it takes closer to 15mins”. The food is highly salty and I never saw any fresh fruit or vegetables. I had to take fruit and packaged oatmeal cups to her So she would get food she could eat. Serving adults hot dogs, warmed up frozen waffles (the fruit with it was a scoop of cold apple pie filling) and other canned and frozen food service items you would expect to find at a middle of no where summer camp is completely unacceptable for adults in rehabilitation that need nourishment to to build strength. The room was never cleaned and dark. The blinds were broken, light bulbs were burned out and took a number of days to get replaced and fixed. The toilet in the room backed up and the staff told my mother and her roommate. “This happens a lot”. They left wet towels in the restroom for 4 days. My mother was supposed to have her bandage changed twice a day and they only changed it once over 3 days. Pros:The Physical Therapy was great and she really liked how they cared for her. Her overall experience was a nightmare. She ended up with a significant infection and had to be rushed to the ER and have another surgery. We refused to let her go back to Avamere.
