I'm familiar with this facility because my mother resided there under Bradford Oak. Nurses/Reception are still there and it was truly nice that they remembered me and my mom that they took care of. The facility looks great and everyone was professional. I have a friend there that just happened to be in the room my mother resided in and transitioned. The moment was special.

Terrible experience for my loved one who left by ambulance in the nick of time. They received very poor quality of care that caused a life-threatening situation and never received some of the care packages that were sent. I hope people check these reviews carefully. There’s clearly a pattern.

My sister was there for 2 weeks of hell! No communication or apparent documentation of patient needs. Food uneatable. One more week would have killed her for sure. If you love someone, don’t put them here.

If I could give them 0 stars I would . They dont change bed ridden patients at all. Someone I was close to went here and caught sepsis. The almost died there . Please don’t send your. Family here.

This place is an unprofessional. The health department needs to shut this place down. The person who approve admission should be fired. The furniture is outdate. And the curtains are ripped. Please don’t allow your family member to be admitted to the place .

DO NOT PUT YOUR LOVED ONES HERE!!!! This place only worry about their bottom line while not providing proper care for the residents. The administration does not do its due diligence on anything and prey on residents who don't know their legal and fundamental rights. If they promise something GET IT IN WRITING or it won't happen. They've had employees and residents die or get severely Injured on their watch. I pray that the residents find someplace else to go for their safety.

I have got to say Ms. Jasmine Young was amazing. She Han taken my mom outside. An because moms phone is broken she call me on FaceTime today . My mom can’t talk but I can tell by all the hugs she gave Jasmine that she really liked her. She was so nice. I wish all of your nurses were as nice as her.

This was my First time there and I will say that the staff was courteous from the front door all the way to the room the facilities were clean and I pray that you continue to give good care to my aunt and uncle

We moved my mom here today. We felt so welcome! Administrators, the nurses, even the one serving food were all so very friendly. This was a scary time for all of us but my mom seemed very happy when I left this evening.

This place needs to be continuously reported to the State and the Fed's until something is done. They are billing the insurance companies for services and supplies that are not being provided to the patients. Also, they need to be audited by the IRS as well.

Once Bedford was sold and became Autumn Lake every thing changed for the WorstFood horrible! The building needs upkeep. There is limited funds for activities. I was there for 2 years I wouldn't recommend it at all!

The parking lot is in dire need of work. The pot holds are not only wear and tear on tires, it's an eye sore. My sister is a resident there since 4/22 and I'm a constant visitor at least one to three times a week. Please fix the pot holds and repave the lot. Thanks in advance for taking care of this problem. The Administrator is extremely great.

Friendly staff. However...Self care items were out of my aunt's reach; remote control, water, eyeglasses...were either on the roll away table ( first two items)or on a separate table near the door (eyeglasses)! She currently is confined to her bed following her recent setback, so I feel that they should always have the over bed table right next to the bed (at least) with these items within reach. Also a wanderer, which she and her son told me can and has become violent, even with three visitors in her room insisted on getting in. My brother shooed her away the first time. When she came back she stood by the door even longer to the point where he had to stand in the doorway, with his back to her, to block her view for a while before a staff member called her away. I now fear that my aunt (Caldwell) may not be safe, for she is still very weak from her recent heart attack.
