Update: Family member was taken to hospital today unresponsive and extremely septic from neglect. The hospital team said there is no way possible someone becomes that sick overnight from an infection. They said it’s neglect and to not bring him back to this nursing home ( if you can call it that). He also has Covid because no Covid protocols are taken,the staff doesn’t wear masks or wash hands. My family member is bed ridden because they refuse to follow the physical therapy order so how did he get Covid? This is a place that lets your loved ones lay in the bed in filth without getting meds or care to die while they sit at the desk on their phones with bad attitudes. If you have a family member you want to dispose of quickly,this is the place . Otherwise beware.Filthy and rundown. This place is extremely neglectful. Your family member wont get their meds or physical therapy. Cancer meds are often not given for days with no care at all.Physical therapy order has been in for months but they have don’t nothing. Guess we will call the state once again….

Once again nasty food. Hard garlic bread, hard potatoes. Dirty utensils. Residents just had complaints about the dirty utensils. Why are they serving food on paper plates? Work ethics have gone down the drain in healthcare. Nobody cares. Are jobs so disposable today? No pride just grab that money huh? This will bite people in the as$ soon because we have plenty of people willing to work coming here from across the border.

Update 4/21/23 my uncle did not get his chemo meds once again today. They are supposed to be given at the same time every morning but days go by and they are not given. When my family calls to ask why he is not getting his chemo meds the nurse hangs up on us every single time. Apparently she feels she does not have to pass meds to her patients or answer questions. She will not give us her name to file a complaint against her because she knows she is in the wrong. We have filed numerous complaints with the state. This place needs to be shut down. Btw my entire family works in the healthcare industry including nursing.Update 4/18/23 just found out prescribed medications to treat my uncle's cancer have not been given since March 31st! How is that legal? He has prescribed medications to treat his cancer from his oncologist and they NEGLECT to administer it to him for weeks? The don't follow his prescribed diet which makes him violently ill. They bring him to his Dr appt on the wrong days with no medication lists. The nurse is totally incompetent and never reads the chart. It's also still filthy and smells My entire family will reportig them for abuse and neglect. Stay away!An absolutely filthy and neglectful dump. Days go by and no meds are given. How can you not give meds to patients? The food is processed garbage if your lucky enough to get your tray. The place is filthy,I wouldnt let my dog live there. Ceiling tiles hanging down,the floors are caked with grime,the walls/baseboards have filth all over them,no working phones for years in the rooms, furniture is filthy and collapsing. I could garbage pick nicer stuff. This place is a cesspool of germs and filth. Absolutely horrifying. There is zero excuse to have a place this rundown and dirty. We will be reporting this place daily until it's addressed

Have found a jewel in Erik English. I thank God for Erik's expertise, caring disposition and knowledge of the human body. He built an attachment to my walker aided in my safety and gave me the use of back. All staff and doctors are second to none.RespectfullyJoyce M Jackson

This place is very shabby and outdated, repairs are haphazard. Furniture and beds are very old. This facility is not very clean. Some patients were left in the hall for hours. Commodes are there too.Not a pleasant atmosphere to say the least.

Terrible. No one answers the call light. PT , speech therapy were good. But the nursing care is terrible.

I worked here the place is terrible and needs to be shut down immediately the dishes are not cleaned properly there’s cross contamination in the kitchen there’s rodents (mice , ants , mosquitoes, nets , flies etc ) there’s literally mouse traps in the kitchen the state should do a review of this place again there’s no way this place is still open .

My sister has been there 5 days, can't get answers pertaining to her care. Staff is rude and does not return calls, her room was dirty when I walked in and had to ask nurse to get house keeping to come clean it. Very dissatisfied.

The Administration, Unit Manager and Kim the social worker should all be embarrassed that they are associated with this kind of so called health care. The CNAs and Nurses that took care of my loved one were wonderful. The facility is absolutely disgusting and filthy. Ants crawling all over, rodent traps in patient rooms, everything is so dirty, I’m sure it’s a health code violation. Tiffany the rehab unit manager and Kim the social worker are both cowards. They’ll mess with your loved ones life and then won’t face the music when you call them on it. Tiffany likes to throw you under the bus Kim. (Although with both your attitudes and games you play, you both deserve it).Twice they tried to thwart my loved ones transfer, over money. You should put some of that money into cleaning supplies, compassion and empathy classes for yourselves. I truly feel for families and loved ones who are stuck in that facility. Karma is real and I hope you get the same experience as your patients do. Because you both deserve it.

Terrible and inhumane neglect. Needs to be constantly monitored to make sure my parent receives medication , grooming and toileting. Wish I can give them Zero stars. Do not send your loved ones here .

We transferred a family member to this facility. It was out dated/broken down and filthy dirty. Some days the smell was overwhelming. There was hardly any staff. If they came to work they did very little. They didn’t do laundry as we were told they would. We had to take the laundry home every few days. Brand new clothes, socks, his winter coat, personal blankets and his shaver vanished. Many mornings he was not shaved, washed or dressed and his room wasn’t picked up or cleaned. He was left in soaked dirty diapers. We had to dispose of clothing that was soaked in urine and feces. We were told there was a hairdresser for the residents, there wasn’t. Most of the staff never acknowledged or looked at us let alone spoke to us. The Nurse Pam and Bessie the Floor Supervisor were the only two people who were consistently professional, kind and attentive to residents and families. We have moved him to a Catholic residence in Naperville. I will miss Nurse Pam and Bessie.

I unfortunately would never recommend this place to anyone. My grandma has had so many problems here. It got so bad we had to move her somewhere else. From employee's unplugging her phone to charge their own, going through her drawers to find food without asking and mind you we always made sure she had snacks to share with everyone. To having an employee put her to bed at 5:30PM without dinner and when grandma told the CNA she didn't have dinner that CNA told her she didn't care. She has had 2 horrible falls 1 of them she hit her head and was laying in her blood for about an hour yelling for help. The other fall she fell out of the sit to stand lift, we later found out there was only 1 staff member operating the lift when there was supposed to be 2. Residents wait a minimum of 45 to 2+ hours to go to the bathroom. My Grandma's clothes were always missing and we had a sign on her closet that the family does the laundry all they had to do was put her dirty clothes in a bag we provided for them and they couldn't even manage that. There were times we would get other residents clothes. This place is so under staffed it isn't funny. Trying to get ahold of anyone from social worker, recreation, administration, etc is a joke. I can't tell you how many times administration would have a nurse call us instead of them or our called and emails would never be returned. The food I'm not sure is even eatable, I wouldn't even feed it to my dog. They said they would have Grandma in physical therapy and there wasn't any physical therapy being done, in fact I don't even think they had a physical therapist coming in. The smell of the home reminds me of a porta potty you get hit with that smell the second you walk through the door. The amount of staff that actually cares is severely out numbered by the ones that do not care at all. So many times my grandma has called us up crying because they wouldn't listen to what she needed help with. If you love your family member, or not, I would highly recommend looking somewhere else. If I had to justify the 1 star it would be because of Nurse Bessie, Nurse Kim, hair dresser Cynthia, Nurse Diana and the few CNA's (I'm sorry I don't remember our names) that would go out of their way to take care of grandma. I could sit here for days and explain to you why not to send your loved ones here.

This place definitely needs to be reported to the Board of health & shut down. After being in the hospital for 6 days, I went here hor rehab & therapy.After 3 days I tested positive for covid 19 along with several other people. They put us in isolation which was understandable. We got treated unfairly. They never fixed my call light, had to.beg for water? & the wheel chair They gave me to sit in was broken. An accident waiting to happen. A resident fell in the hallway. If my roommate hadn't used her call light, who knows how long he would have been lying on the floor.Another issue I want to bring up is this: I had a fever of 103 & Cindy the therapist black balled me because I was too sick.to do therapy..

This place is absolutely one of the worst places that we’ve ever had to deal with I’ve spent the last five years dealing with rehab centers and nursing homes in the area between my mom and dad my mom’s recent stay in this place has been nothing short of pathetic if you were a dog at a dog shelter they would treat you better every request made everything that we specifically needed done never got executed properly I could go on for days all I can say is do yourself a favor and take your loved ones somewhere else

My mom has Dementia and other health issues. My family and I made the difficult decision to move our mom here and we have never been happier. Though for several months it had been difficult not seeing mom due to the COVID-19 lock down & CDC guide lines the facility had to follow. However Palos Rehabilitation Center during that difficult time arrange for family members to talk/see love ones a couple of times weekly via FACE TIME!Mom grooming/hygiene appearance was great! Mom looked good & expressed she was doing okay.Now family is allowed back in to visit Love ones, mom looks great, and says she is happy!The Palos employees who worked with mom are very nice & professional.The team keeps the family updated on mom’s overall health needs. I have called or email the social worker, business office, Nursing team, mom CNA’s Physical Therapy department,Activity Department, and or spoke with team member caring for mom ,this also includes the dietary department .Moms CNA’s are very nice, patient and attentive towards mom. The team member listed responds right back!Family members before restrictions were put in place again, we were able to monitor mom eat her meals.The meals are nice healthy portions.Mom says “delicious “!Wanted to also thank the House keeping department.Mom room, & bathroom laundry,& linen on mom bed is always white/clean!Thank you Palos Rehabilitation Team for your continuous support/supervision to our mom!Sincerely, Lynne Dorn & Linda Czarnecki (daughter’s)
