“We just recently started using Transitions Hospice for our Dad who is in a Memory Care facility. So far, they have exceeded our expectations. Everyone has been very responsive, thorough and kind. The nurses and CNA that we are working with are really great and have common sense, which is hard to come by sometimes. They proactively realized that it is best to try to keep the same person visiting for a dementia patient. Their top goals seem to be compassion and quality of life.”
