They took good care of my mother in law and were very kind. Staff was great and loved the garden area. They did lots of activities with her.

Creekside took great care of my mother during her short rehab stay. The staff was friendly and worked hard to ensure she could return to her assisted living facility as soon as possible.

Bring your loved ones here if you want them treated like cattle.The workload brought onto the workers here is crazy. The residents are left in their filth and things are shoved under the rug when management is made aware. I witnessed not only neglect from the CNAs but also the nurses giving a resident another resident's medication on accident. When family is set to visit, everyone rushes to "clean up" the patient so they are presentable for the visitors. Don't work here and DO NOT put your family here. I quit after 3 months due to the neglect and lack of care at Creekside.

I worked for creekside since December of 22. And the rotation of hiring is nuts. They will hire absolutely anyone and when the person was neglecting the residents they wouldn't care. You practically work a hall by yourself if you don't have a strong person when a hall should honestly have 3 aides. You are told you get a lunch break but no one is able to take one because you're to busy. They treat their staff not the best and when confronted about it they say they will fix it and they won't. I do not recommend creekside. Not high quality care. They make sure the rehab side is staffed so they have an easier day (3 aides always ) and that's only because they get more money from the rehab side than the long term care side. They bribe staff to write good reviews by giving them T-shirts. If you care about your family members do not send them to creekside. The management team will work you til you drop. And when you have concerns about a resident you are pushed to the side.

I really enjoy working for Creekside! They really do appreciate their employees and makes sure residents are always well cared for. I hope to eventually become a nurse here once I complete my PN Program! I am currently a CNA.

My mother seems to be happy at Creekside and I have no complaints and haven't heard any complaints so I'm happy with Creekside as well!

The nurses are great. Food is good. Activities are fun especially the ones Madison and Taylor create for us. garden club, and women group always have something new all the time. nurses all well trained and very kind. Kitchen has an alternative menu so we always get something we want to eat. I would recommend the therapy. When I first came here I had a broken hip and could not walk. With their help I can now walk. 5 stars from me.

We had a great experience at Creekside! I am thankful we had such a caring nurse like Megan to care for my mom during her stay! The therapy department was such a delight and very knowledgeable

When I arrived at creekside I could not walk. I am now walking with a cane, I am in above the knee amputation I have a new prosthetic leg , my therapist McKenna is the most professional individual I know he takes time to make sure that I am adequately prepared for my next step the nurses and CNAs at creekside, are professionals. The people in the administration treat me with the utmost respect. Since I've been at creekside I was elected president of resident council , I would recommend creekside transitional care to my family members and friends that I know

The Director The Director Of NursingTheir Door Is Always Open If You Need Them, Or Talk. Good Choice Of NursesAnd CNA’S They Try To Be So PleasantEven When Busy. And A Good Response To My Alarm On 300 Hall When I’m In 401. I Always Introduce Myself To New Ones And Establish A Good Repore And They Know What My AIarm Sounds Like! I’m Pretty Self Efficient Even Do My Own Trash And Water At Night. Just To Help. Love My Room And Sometimes I Make Everything With A Smile! I Am Vice-President Of Resident Council AndGoing Well lol Everyone Pretty Much Knows Me Since I’ve Been Here The Longest! They Like Me And Feel They Can Talk To Me!! Such A Blessing To Them! I Try To Be! This I Hope Is Like Go Between And Helpful Before Having To Go Any Further And If SoWe Will Know Who And When!My Son’s Are So Appreciative To Creekside And When They Know They Can Call Director Trent And Speak Such Kind Comforting Honest Words!I Reccomend Anyone Should Please Come Ser Creekside It’s Like 1 Big Family Oriented! I have debated writing this, but I could not be quiet about this place. My Ma passed in her home this week after being emergency discharged from Creekside (at my insistence). Her care while at this facility was less than stellar to be as kind as I can possibly be with my words.The Physical therapist was all too happy to mark her down as a "refusal" when in reality, she had asked them to wait a moment while she finished what she was doing at the time.The doctor refused to alter her medications at my request as her pain level was NOT adequately managed.They purposely canceled her oncology appointment saying "that's not why she's here, we're not going to transport for that". She needed that to establish care for her cancer, and get approved for hospice.My mother had quite a bit of money and I believe they would not allow her to establish care with her oncologist so they could delay her discharge and juice her for money. They charged her $1400 a WEEK to stay and not even get adequate care.Once she was "prepped for discharge", they entered the order WITHOUT a safe home environment to discharge to. I had to put her into their LTC unit.She was wheeled into a room of a woman who had been there for 8 years and had her things all over the room. They also only put a curtain up to separate the space. They left this other woman's things so my ma had no personal space of her own. While I understand it's sometimes normal to put residents in the same room, what they had planned on charging her ($9k a month) was plenty for her own room. They dumped her stuff in boxes and left her in her wheelchair. Did not make her bed or get her into bed. She was a hoyer lift full assist patient.There are so many things wrong with creekside. After her first night in LTC, I received an email from her stating she was willing to stand on her own and "crash and burn" just to get out of there. Between the yelling and full-volume of the TV, it was mental and auditory assault all night.Absolutely nobody should be subject to that sort of indifference and mental torture.If I can prevent ONE person from suffering in this hell-hole, I will do what ever I possibly can to save someone's loved one. Ma also had clear signs of advanced dementia. To which staff fluffed off as "she passed our"cognitive testing" knowing what you want for lunch is a far cry from being able to make good decisions in your healthcare. She was consistently making poor choices. I had removed myself as her emergency contact, yet the facility had my brothers information and brought NONE of this to his attention.Ma had an emergency transport out of there Thursday afternoon, and passed away in her beloved condo the following Tuesday. I one hundred percent believe this facility and their poor management, administration and staff contributed to Mas rapid decline.Save yourself a lot of heartache and pain and❌❌DO NOT PUT YOUR LOVED ONE HERE!THEY ABSOLUTELY NEED TO BE INVESTIGATED AND SHUT DOWN! ❌❌

I was very pleased with the care and Rehabilitation that my brother received after his fall, breaking his hip and elbow. He exceeded all expectations with his recovery in rehab with Ben and Gretchen.My brother has dementia and they, along with the entire staff, quickly gained his trust even though he still had brain fog from his surgery.A bonus to us was the kindness and compassion that everyone we encountered gave the family.We had previous experiences with rehab facilities several years ago as my parents needed it and this place made the others pale in comparison.WARNING: when you check your loved one in they tell you that they will give you our 48 hour notice before termination of services but they mean ONLY 48 hours of notice and no communication a minute before.

I came to work here about two months ago. I absolutely love this place. The residents are happy and well cared for. The employees that work here are friendly and truly care about the people that reside here. This is evidenced by the way they speak them and the care they show at the bedside. As a nurse I appreciate the communication that goes on continuously in an effort to make sure that staff get information needed to do their job and do it well. In the short time I’ve been here I have witnessed and been a part of communicating with residents and family members to ensure that they know and understand any new order. Granted, I work here but I would gladly recommend Creekside Transitional Care and Rehabilitation to anybody looking for a place for their loved one. While here you can expect to receive top notched Physical, Occupational , Speech Therapy, and be treated like a family member by the staff that work here.

I have worked at several facilities over my career and Creekside is by far the best! I would recommend it to any friends and family for rehabilitation and/or long-term care. The staff are wonderful, the food is great, and the therapy team is top notch!
