My father was here for a short stay. Kamani the nurse did a great job of taking care of him and during her shifts his care was adequate. Unfortunately care was not good across the board and there were other times when I walked in to find him soiled with no one around to help change him. My mother and I cleaned him up and changed him several times ourselves while waiting for people to help after pushing the call light. Additionally, One unsafe situation occurred last night when I came to visit seeing him slumped to the side in his wheelchair and extremely drowsy. I talked to the nurse who admitted to giving him trazodone and other sedating medications. I informed him and staff that is unsafe to give sedating meds and have them sit in the wheelchair without locking it, and he very well could have fallen flat on his face. He should have been put in bed after giving the meds. Please make your RN’s and CNAs aware that this needs to happen for safety if giving sedating meds like benzos, trazodone etc. I am giving 3 stars and not less only because kamani and a few other staff members were pleasant, helpful and did do a good job with his care. This was certainly not across the board though.

Lmao this place is horrible the cnas dont care about they patients, they let patients be by themselves when they need help, 2 the admin here is horrible they let the the staff treat everybody horrible, everyone here is biased especially eloise daughter, ms gurl needa get some d in her life cause she too focused on one person! LMAOO dont bring your family here!

My sister currently resides at the Guardian Care Nursing facility. She has chronic illnesses requiring rehabilitation and observation. One of the nurses discovered my sister unresponsive. The nurse immediately called emergency services and she was transported to ORMC hospital. The E/R staff stabilized my sister. The quick action probably saved my sister's life. I'm very grateful to the Rehab nurse prompt, medical response!

My mother was sent there in early June for rehab for broken ribs and vertebra in her low back. My siblings had toured this facility beforehand and it was quiet and clean. When our mother arrived at this facility, it was awful. They left her in a wet diaper all day/night, her meals consisted of high sugar foods/drinks (she is a diabetic). She was never evaluated, never got PT, just left there uncared for. The nurses do not care and have no compassion, some do not speak good English. My mother was constantly told to wait her turn to be changed and for water! I think animals get treated better!!! After 3 days, we removed her... They had 27 active Covid cases after boasting they had ZERO!!! There were also screaming old people at night and when my mother would inquire about them, they told her to shut her door and ignore it! She got no rest in this horribly run place!! DO NOT LEAVE YOUR LOVED ONES IN THIS TERRIBLE PLACE!!! NEVER AGAIN!!

After carefully visiting and researchingover eleven Nursing homes and assisted living facilities I chose Guardian Care to take care of my love ones. They have proven to be absolutely five star under administrator Eloise Abrams. Thank you from the bottom of my heart ❤️

This facility is horrible. My mom was sent there for short term Rehab. I had her removed in 2 days. They would leave her in a wet diaper all night and when she would ask for a glass of water, they would tell her she had to wait her turn! Needless to say, the water never came. Old people would be screaming all day for somone to help them. This place is horrible and I would not leave my dog there!!

Awful experience! My mother has several medical conditions which left her unable to take care of herself and needed rehab. In my opinion, this was one of the worst places for her to go. The facility is really nice but some of the staff need more training on how to care for the patients. It would take over an hour at times for my mother to get assistance when she needed help, they never got her out of bed to help her shower, and very seldom would they take her to uses the restroom/bathroom. I thought this was part of rehab? I observed so many issues with this facility. And when my mom and I complained about an incident that took place with one of their staff, they quickly tried to cover it up and act as if it never happened. I would never recommend anyone to this facility. We were verily disappointed. :-(

I arrived here on July 8th 2021, While at ORMC trauma for four days I did my research and found Guardian care new facility. I spoke with Julian who was also a Army veteran, I told him of past experiences of prior stays at a rehab closer to my house, he told me sincerely that I would be in good hands and he would see to it. From the cna's to the nurses to the dinning, all staff, directors, PT who are really good at what they do, to laundry, recreation, they all work in harmony in making sure my and others stay is as computable as possible. They have great security and gated, which helps to keep any rift raft out. I believe Julian gave me good honest advice and believe he is as well as others are a big asset to this company and I truly whish it was close to my home but sometimes you have to cross the river to get good clean, honest, professional rehab. I feel I am not just another number but I have a name and I'm getting to know all and feel very comfy and my big thanks to each and everyone of you in my health and rehab. It takes a team and unity and I feel blessed. James Tyler

As a family member, I will not recommend this place!!!! My father is a retired VA Army man who served over 30 years and I couldn't believe they made false statements about my father and told us they need to call 911 just because they saw something wrong in the xray. The ORMC ER doctor was very upset by their false statements when the hospital checked everything before they send him to that nursing home. What made them say something like that was what the doctor asked. As a family member I have called several times yesterday to speak to a caseworker and all i could do was leave the message. The caseworker NEVER called back! The receptionist told me that the STATE PEOPLE are here and thats why she couldn't call back. Today is Thursday and i still havent got any calls from them! The congressperson and other higher levels will be hearing this matter because Guardian Care Nursing Home discriminated a RETIRED VA ARMY MAN!!!!
