I have been coming to excellence health for a few years now. The staff are very friendly and i love Delayna. My husband and daughter and I all come here.... when i have a concern about abything with my health she listens and does everything she can to Accommodate our needs... weather its a referral or something she can do... I love how personable her and the staff has been It i could guve 100 stars i would... I highly reccomend anyone to go here.

I was on the fence about giving this group a bad review , Ive seen a lot of negative experiences written on here about them.My advice : Avoid going to them . They are least interested in spending time on patients, just prescribe the medications and go away. No explanation, nothing. goal seems to be maximizing the number of patients seen and revenue. I received a bill for services that we never received in highly unhygienic and crowded nursing home ( They Call it Post Acute Rehab ) for my dad who died because spinal cord abscess in near hospital after they recommend hospice service for him, this is the end of their money making chain . I dont know how they are in practice medicine , non of them are board certified. We are going to take a legal action soon after this disaster in my dad medical management.

I love the team at Excellence Health. Gloria is so personable at the front and eager to always help. Delayna is who I see and she’s no frills, no fuss and talks with you (not down to you) about your health! And encourages you to ask questions!

The team at Mason Ave is the absolute best! It all starts with Gloria who is the most professional and efficient staff member I have ever encountered at a healthcare practice. Gloria has a unique set of interpersonal and business skills that serve both patients and the healthcare staff well. She is an outstanding resource. Dr Delayna's expertise and compassion for her patients is exceptional. I have rarely received such outstanding care from any medical professional and she is the kind of person that understands how important it is to be the patient’s advocate. She listens to you and applies sound judgment to address your needs. I would recommend this practice to anyone. Excellent healthcare is not just in their name; they actually provide it!

I have been coming to this clinic for over 2 years now and have always felt welcome. They discuss everything with me and we work together on what medications I need. The office staff is always welcoming and make it easy to contact them about any issues I may have. I am thankful to have a doctors office I can trust.

Our APRN Delayna has been a true blessing since my wife had a severe stroke in April of 2022. Recovery is based on the ability of professionals to provide whatever it takes to accomplish a goal regardless of obstacles. In a long term recovery program it is crucial that all the elements come together to assure success. If I could have rated this awesome service we have received higher than 5 stars it would be done without hesitation. Their dedication has been phenomenal.Thanks for amazing results.

Excellence Health is just that. Excellent health care! I feel they have brought back the human side of medicine. The ladies in the office are so caring and thoughtful it makes you feel you are getting the best care from very one in the office. the Doctors actually care about you and your health! And everyone in the office puts you and your health first! It is amazing! Dr Chavez is a super knowledgeable Doctor and he cares about your health, he actually cares and asks questions and counsels you to be as healthy as you can be and how to get the best health you can have. Excellence Health has brought common sense back into medicine. I love this office and I feel that they really care about me and my husband. I wish all doctors practice this way.

Horrible experience. Dr Chavez is very unprofessional. My husband was in a motorcycle wreck and had a huge hematoma. We needed an mri done for insurance purposes and so dr Chavez scheduled the mri but for the wrong location and when asked about it, he just walked out!
