Some context: I was a patient of Dr. Tremaglio pre-pandemic for a couple of years or so, and before that, I was a patient of his colleague, Dr. Post, throughout my entire childhood for my chronic cystine stones (cystinuria). Starling Nephrology has a chart for me going back 30 years; the diagnosis remains the same, as does the standard course of preventative treatment.
After Dr. Tremaglio failed to refill my prescription back then for reasons unknown, I decided to just go off the Urocit-K for a while due to a combination of pill fatigue and the fact that the medication didn't historically have much of an impact on my stone formation.
Fast forward to now: after a couple years of forming stones at a much faster rate and needing several surgical interventions, my urologist urged me to reconnect with a nephrologist and resume treatment.
They booked me out two months, and when I asked if Dr. Tremaglio could call in a Urocit prescription in the meantime to hopefully keep me from growing another massive stone while I wait for my appointment, he refused.
The nurse I spoke to suggested I contact my previous provider for a prescription, and when I told her my previous provider was in fact Dr. Tremaglio himself, she went back to him with that, and he still refused. Her rationale was that since it had been a few years, the dosing might have changed (there's actually a standard starting dose for adults), so she suggested I request the medication from my PCP. Predictably, she did not have a response when I asked her if my PCP, who is not a nephrologist, would likely know what dose to prescribe.
I'll note that the medication I requested is potassium citrate, which is naturally occurring in citrus fruits and even used as a preservative in some sodas, and it is the first automatically prescribed treatment for cystinurics to boost pH.
To sum up, Dr. Tremaglio is "uncomfortable" prescribing a harmless medication to a patient that his practice (and he himself) has seen and prescribed the same medication for going back three decades. If I could, I'd tell him directly that I'm uncomfortable having cameras and laser fibers jammed up my urethra every six months, but I understand sitting down at a laptop for 90 seconds and ordering a script is just a bridge too far.
So much for the Hippocratic Oath, right? Thanks for reading -- I'm off to chug some lemon juice so I don't lose a kidney before I'm 50.

Dr. Joseph Tremaglio is such a kind hearted smart man who knows his specialty and then some. If you need a nephrologist who takes the time and consideration of your history from birth till the age you are at now. He is the one for you. I cannot say enough good things about him.
