not a good place my father was just in there and was not treated right was denied his meds and dialysis and was not given the right care will not recommend this place and had another patient next to him and was a stroke victim and was getting bed sores and would not even help him get up to eat would wait in hour for an nurse to come help him eat his meal and my father had to wait to get his bath for almost a day he had to try to walk after his surgery to clean himself cause of failures nurses not doing there job would not consider this place for any rehab for patient to get better go to a different rehab center to get the proper care cause this place doesn't do that

My grandmother is currently receiving care here and all I can say is that they are phenomenal! She suffered from a NSTEMI heart attack back in September and also had to undergo emergency dialysis at the same time while at the hospital she was admitted to, then was discharged to Valley Healthcare Center where she was horribly neglected and even obtained state 4 bed sore and severe fluid overload, we discharged her from there and had her transported directly back to ER where they stabilized her and she's been here receiving PT, OT and speech therapy.Everyone from the security, front desk, admissions, CNA's, nurses and physicians have all been above and beyond amazing! The facility is absolutely spotless and excellent in cleanliness. Their PT/OT room and equipment are wonderful and you always see the therapists working with the patients in and around the facility, if you're looking for a facility that cares about their patients this place is it. I'll update once she's been discharged for how well case management and social services are although I’m sure they’re great as well.

San Bernardino Ballards. Frank,Nia,Chantel,Beverly, Big Ben, Edith, Vanessa, AJ, Jonah, Mia, Macario and lets not forget Lorena, Luis respiratory therapist and my physical therapist RK, Marissa, Nickie, Nichole, Mary all of them are great best ever but Evelyn is the bestest ever. Her and her baby partner are awesome they all(everyone list) handle their task with a great attitude and smile. It makes writing this review so easy even feel like im forgetting someone which i apologize now if i did. They know what what they are doing and what needs to be done and im forever greatful for them and their attitudes. Im still here so i still have work to do and im glad im here and wouldn't have choosen another facility with better ratings. That 3.9 doesnt do them justice and its clearly a typo. Someone just being a complainer and doesn't want to put anywork into their own recovery but enough about them the negative ones. And another thing the food is not bad at all ive been eating everything which is rare. Im looking forward to my full recovery which i know now that it will be sooner then later. What im not looking forward to is having to say farewell to them when my time and recovery have been completed..so i say to them all from the bottom of my heart muchas Gracias and i appreciate you all more then i can express. And to Paola y Brenda the new rookies on the block you young jalisciense gals are off to a great start, fabulous attitudes and willingness to learn and get your hands dirty are gonna make a lot of patients willing to put a lot more effort into their own recovery. Great motivators . And all my nurses have been beautiful inside and out. With love and respect forever grateful Ernesto Orozco

Ballard Rehabilitation Hospital holds the gold standard for successful treatment and outcomes of debilitating diseases and serious injuries. The Team at Ballard are led by a wonderful group of professionals who share genuine compassion and care for their patients and families alike. I have appreciated many positive experiences with Balllard and I highly recommend their specialized therapies and programs.

Staff and nurses were rude and bedside manner was terrible!!!!! The only person who showed compassion was Dr.Chen and he deserves better than this place!! My grandmother was having a seizure and another doctor (who wasn't doctor Chen) said she was stable while watching her have the seizure, doctor Chen comes in and notices the decline in her condition and sent her to the hospital right away!! I would recommend going somewhere else for treatment!!!!

I was a patient at Ballard for 3 weeks due to Cancer treatments and to regain my strength to walk again. The Nurse's was very nice and thoughtful, and the Physical Therapy was AWESOME ! I loved all the therapist.. I would recommend Ballard , I had been to other facilities for help, but Ballard's was by far the best.. I had many other health issues and the nurses and doctors addressed them all. Once again Thank you to the staff.. Debra Butler 104 W

Negative zero stars from our family. It is my hope that the Director Melissa Curry see my review and contacts me. As I will be preparing a formal written statement. My Dad arrived at the facility a week ago. They did not do his intake properly. Paperwork was signed and dated 3 days later without my Mom present and without the proper personal information. Nurse Mikaya has personal issues and she speaks with colleagues about her personal problems in the patient’s room while giving care to a patient. When a patient is afraid to speak up and ask for help due to the “nurse” being tired or upset that is unacceptable. My Dad’s wound is not being taken care correctly. My Dad feels uncomfortable and unsafe saying anything due to the nurses seeking retribution. My family is concerned the nurses will seek retribution on my Dad for us speaking out. The facility hides behind HIPAA when it’s convenient for them. Yet they talk about patients, colleagues and other irrelevant not work related drama at around patients and in ear shot of patients trying to heal. Charge Nurse Susan claims to follow rules and regulations. Yet She does care about the patients she only cares about herself. I have spoken to Justin and Erica who are supposed to be case managers while they are friendly and give an ear they have no power over the nurses or actual care of the patient. If my Mom isn’t there everyday to make sure my Dad’s wound is cleaned “properly” it won’t get done. My Dad went there with negative infection cultures and now he is positive. While I understand there is a 2 sided stories. One cannot get honest and clear information when it is requested. The nurses chart so that they are not held accountable for their actions. They will hide behind HIPAA and that is ok. Because i will stop at nothing to protect my Parents. If you send your family member there. Be prepared to be present very attentive to your loved ones care. There are so many more issues. Just beware that this facility presents itself as wholesome but it is not. There are a few who were true to their code of ethics. Nurse LeeAnn was courteous and informative to my Mom. CNA Malary was kind and helpful. Marjory the front receptionist is always friendly and welcoming. Even when you have to call back several times. CNA Ana was also very kind, helpful and respectful.

I was referred to Ballard rehabilitation... For physical therapy by my doctor.. I usually go somewhere else., But I needed a closer location. Then the usual spot that I go to. I am so glad I was recommended to this outpatient hospital for physical therapy. Since I've been coming to Ballard for my physical therapy. I've had great experience with the physical therapist... all of them... They are so polite and Attentive to my needs. My 3 favorite physical therapist, are Lory, Matthew and Emily... Actually all the physical therapists are so great but Just can't remember all of their names.Ha ha ha ?Anthony the receptionist/ Scheduler is the number one Person that you will be in contact with for scheduling. I really love Anthony's positivity about everything. He is so nice and always has a smile on his face. A smile that will make you smile. And will surely Brighten up your day.Really. Enjoy coming to Ballard for my appointment.Thank you.Beatriz G.

I have been at Ballard just over a week. The whole staff is amazing. Through their help and encouragement, we have accomplished so much. They work with you every step of your journey of rehabilitation. Trust them and the process. They are knowledgeable and I highly recommend them. Thank you to all the staff ?

My aunt has been here for 2 weeks now and just like two of the staff members have been amazing to my aunt. The other staff members are very rude especially a doctor , he treated my aunt so bad and is just sarcastic over serious things . She just got her leg Ambutated and the way he took her thing that is wrapped around it was so brutal and he laughed because she was screaming of pain. That specific doctor should not work here and from his supposedly 32 year experience he has not learned how to treat elderly people who are in pain. You never know something could happen to anyone and if it was his family Ik he wouldn’t want anyone to treat his family that way. Thank you for those who actually treated my aunt properly

My sister stayed here for two weeks after a back surgery. They took good care of her here. After she was discharged she could have continued physical therapy at other facilities closer to her home, but it's been 7 months and she continues to renew with them because the staff truly care and work hard to get their patients back healthy.

BALLARD AND THEIR TEAM ARE HIGHLY SKILLED,COMPATIONATE AND FOCUSED ON THE PATIENT. I AM SO BLESSED TO HAVE HAD THE EXPERIENCE. They are like family and are kind to each other. Hard workers. All if them desire to progress in their skills and training. The facility is clean and well taken care of.

I would give this facility a ZERO if possible. My very close family member has been here for about a week and a half. Management is poor. The nurses are not taking proper care of wounds, and dressing care. They had not cleaned my family members wound in days. I then followed up to ask for his nurse to clean his wounds, I was then shoot off. I was told she would get around to him. We waited. We asked if we could get help putting him into his bed, they took almost an hour and a half. Still, no one showed. He ended getting into the medical bed alone. No one still showed, he was in pain & due for his meds. NO ONE SHOWED. I am very disappointed in this facility & will never recommend for anyone. I understand there’s other patients, but treat them as if they were your own. Be passionate about your work, and professionalism. I hope no one has to go through the poor care that these nurses provide. They did the bare minimum to keep him at least comfortable.. Zero stars. Do NOT come here if you care deeply for your family members.

Physical therapy is awesome . My mom didn't want to leave . They are staffed with travel nurses and I'm house staff . Case management is iffy. However CNAs are awesome , the doctors are amazing, and PM shift LVN and RNs are awesome shoutout to AJ , Lonica, Mei , Christian , and Aja (your bougee girl misses you ) !!!! I wish my mom could've stayed longer They're very attentive to patients , and they truly care here .
